UPDATE: Registration is underway for second COVID-19 vaccine POD here on April 7

Anyone age 30+ and age 16+ with certain medical conditions are eligible to register for the second COVID-19 vaccine POD on Wednesday, April 7 from 9 AM to 5 PM at the Shelter Island School delivered by Stony Brook Southampton Hospital and the Town of Shelter Island.

[UPDATE: As of Tuesday, April 6 anyone age 16+ can register. Also, there is a waitlist and in event that extra doses are pulled from the vials, the organizers want to be sure none are wasted. Sign up for the waitlist and you may be called.]

Here’s the link to the registration form and waitlist. The registration for this POD can only be done through the online form and is only for people who are receiving the first dose of the vaccine. The second dose will be administered at the Shelter Island School on April 28.

First pick an appointment time — the organizers expect to deliver 580 vaccinations — and then fill out your pertinent details, including name, address, phone and the like.

You’ll be asked to confirm that you are eligible. Check the latest guidelines here: NYS COVID Vaccine Certification Eligibility List. After that, you’ll be asked to check these items:

  • I have read the entire list of priority groups eligible for COVID-19 vaccination provided in the link above.
  • I hereby certify under penalty of law that I am eligible for COVID-19 vaccination.
  • I further agree that by clicking and selecting “Yes” and submitting this form, I am placing the legal equivalent of my handwritten signature on such certification.

Spots are likely to fill up quickly, so get started ASAP.

What to bring to the vaccine POD

On April 7, you’ll have to bring to the site your identification and proof of eligibility status by completing the New York State COVID-19 Vaccine Form. You also must complete the Stony Brook appointment CONSENT FORM and bring the completed consent to your appointment.

Masks and social-distancing are required. Please wear a short-sleeved shirt for ease of access to the upper arm vaccination site. You will be required to remain in the post-vaccination observation area for at least 15 minutes after receiving your shot and should plan accordingly. You’ll receive a card noting the date and lot number of the vaccine you received. Make a copy (take a photo with your phone) and then put the original away for safekeeping. You’ll need to bring it when you get your second dose and use it as proof that you’ve been vaccinated.

If you do not book an appointment and/or are not eligible, you will not be vaccinated, the Town says. If you have a fever or are unwell, you should cancel and reschedule your appointment. Contact your primary provider for questions related to health and vaccinations.
